Where did this silly expression originate from?
Okay, I accept that most people only have one head. However, with
regard to restaurant charges etc, why use this term. Would I still pay
'per head' if (for example) I had two stomachs?

Why not ?0 per sep of lips / per nose/per ANYTHING??
